Welcome to

Beaverton School District - Special Education - Transition Services
Transition services are a required service area for students on an Individualized Education Plan (IEP). These services begin when a student turns sixteen years old. In Beaverton School District, planning for these services begins when a student enters high school. Each comprehensive high school has a transition teacher on staff that begins working with freshman students to educate them about transition services.

Freshman students working on transition activities are introduced to a variety of areas related to transition services, e.g., work experience, developing a resume, receiving feedback about work habits, increasing their awareness of the importance of physical appearance on the job, and other issues. In-school work experiences are provided for students so that by the junior year, job skills have been developed that will allow students to be successfully employed in the community.

A transition plan is written for the IEP that will be in effect when a student turns sixteen. It is an integral part of the IEP and includes supports in the areas of Personal Life, Employment, Graduation/Post-Secondary Education/Training and Daily and Community Independent Living. The IEP may include specific goals in one or more of these areas, as well as activities that the student and family will complete in order to make progress toward the student's post-secondary plans. Each transition plan is individualized to meet the needs of individual students and is updated annually.

  • Personal Life Activities: This includes hobbies, interests, activities or sports that the student will participate in, as well as skills in developing relationships, decision-making and problem solving.
  • Employment/Career Activities: Work experience, volunteering, part-time and summer work opportunities, as well as skill acquisition and career exploration.
  • Graduation/Post-Secondary Education/Training Activities: Students discuss their high school graduation plans, which include post-secondary training opportunities.
  • Daily and Community Independent Living Activities: Learning to use public transportation, money management, cooking, cleaning, and other independent living skills.

Welcome to

Cascadia Behavioral Healthcare
PUTTING COMPASSION TO WORK.
Cascadia creates hope and opportunity for people with mental illnesses and addictions in Oregon.

We continue to make progress in strengthening our financial and operational performance. A major restructuring of our senior management and fundamental improvements in the way we do business means that the clients we serve can continue to expect quality and compassionate care.

Together with our collaborators in county and state government, and with our community partners, we’ll continue to provide a web of support for Oregonians with serious mental illness and addictions . Our primary objective is to contribute to every individual's best possible health and freedom from addictions, and consumers tell us we're succeeding.

Many challenges remain, and we're up to the challenge. We continue to streamline the organization, focusing on those activities that are the best fit with our core mission. We hold high expectations for our new leadership who've empowered the organization to effect needed change. We’re instituting new state-of-the-art business practices, including a continued rollout of our new electronic billing and medical records software. We’re doing what it takes to be successful.

We're knitting a safety net which is far from complete, yet fully in use. People facing mental illnesses and addictions in our community have good reason for hope, and we're part of it.

Welcome to

CODA
Providing evidence-based addiction and mental health treatment for the greater Portland area.

CODA is a behavioral healthcare agency. Our mission is to significantly reduce alcohol and drug problems and related social, health and behavioral consequences. When people come to us with alcohol or drug-related problems, they are often at a crisis point. They may be physically and emotionally ill; their relationships, financial situation and work lives can be dysfunctional. They may be in trouble with the law, without homes, or lacking other things basic to making their way in the world. At CODA we work with women, men and families as they take on all of these challenges. We provide the support and treatment needed for them to reduce and eliminate their alcohol and drug use; reconcile with family and friends; find employment, and take a place in the larger community.

CODA has been building stronger families, safer neighborhoods, and healthier communities since 1969 through comprehensive behavioral-treatment options for people in need of substance-abuse or mental health services. In 2005, CODA expanded its mission to include treatment services for those struggling with severe and persistent mental illness.

JOIN
Connecting the street to a home.

JOIN exists to support the efforts of homeless individuals and families to transition out of homelessness into permanent housing. Our efforts are directed at individuals sleeping outside or in their car in the Portland Metro area. Our service provision is not dependent on age, gender, ethnicity, sexual identity, specific diagnosis, or identifiable issue.

Values & Principles

  • JOIN always has been and always will be organic—living and evolving—because no one has all the answers.
  • Organizational simplicity—because responding to the complex issue of homelessness requires flexibility and creativity.
  • JOIN's philosophy is one of inclusion—restoring connections and rebuilding community—because the experience of being without a home is of one of alienation, degradation, and dehumanization.
  • Homeless people themselves must be the primary agents of change—because paternalistic approaches are devoid of respect and devalue the strengths of the homeless people upon which long-term stability depends.
  • Relationships rather than rules—because in the end, it's aboout the people.

Welcome to

Nonprofit Network Southwest Washington
We build community by strengthening nonprofits. Nonprofit Network Southwest Washington is focused on creating an infrastructure of support for local nonprofits in a rapidly growing and dynamic community. Now is exactly the right time to connect the dots of local organizations. Clark County is still relatively closely connected and has a history of partnership based on strong relationships. However, the influx of new people, new organizations, and new needs requires new forums for establishing and renewing relationships. Nonprofit Network Southwest Washington is helping to create those, at the same time we are mapping and documenting valuable work done by current and past leaders and groups.

Portland Community College (ETAP) Evening Trades Apprenticeship Preparation
Program Overview
The Evening Trades Apprenticeship Preparation (ETAP) program was established in 1998 by the Housing Authority of Portland (HAP) with the opportunity to move residents to self-sufficiency as a pre-apprenticeship program to provide introductory construction training and employment support services to begin careers in the construction trades.

Now with Portland Community College, Cascade Campus (PCC Cascade), ETAP continues to serve residents of low-income residents of Multnomah County, as well as HAP public housing and Section 8. ETAP participants must be at least 18 years old, have a High School Diploma or GED, and have legal documentation to work in the US. Participants receive up to 240 hours of classroom and hands-on training in a 11-week training session. The ability to work out doors, do demanding physical work, and possess base-line reading and math skills, as well as pass random drug tests, is required. ETAP staff are available as “coaches” and case managers for participants through out the training and up to three (3) years after ETAP graduation.

Portland Habilitation Center, Inc.
Employing a thousand members of the community Portland Habilitation Center Northwest (PHCNW) is a Qualified Rehabilitation Facility (QRF). PHCNW delivers contracted services in custodial, landscaping, light industry and administrative services. According to the Portland Business Journal, PHCNW is the sixth largest non-profit organization in the region. PHCNW is not affiliated with any social service agency. Funding from government sources constitutes less than 1% of our annual operating budget. PHCNW operates on a sound business principle: “Deliver consistent high quality reliable services and the work will follow."

Our mission is to assist individuals with disabilities by providing the training they need to develop their skills so that they may obtain and maintain meaningful work.

Beyond Limitations™
The growth of PHCNW is rooted in the individual commitments and efforts of our workforce. By encouraging personal initiative, our theme, “Beyond Limitations™," occurs when individuals achieve the unexpected, continuously improving their work performance and delivering value to our customers. Our success is anchored in quality checks of activities and processes, as well as ongoing training.

Promoting Diversity
PHCNW serves individuals with a diversity of disabilities. Many non-profits are able to assist workers with only certain types of disabilities. Many rely on case management fees from families and agencies to fund their work. Over 25 years ago, PHCNW chose to take a different approach in order to provide jobs, not just advocacy. Our work is based on providing opportunities to succeed and become contributing members of the community—earning living wages, paying taxes, raising families and living independently.

The YWCA is the oldest and largest multicultural women's organization in the world. Across the globe, we have more than 25 million members in 122 countries, including 2 million members in 300 local associations in the United States. More important than the numbers, is our mission to eliminate racism and empower women. We provide safe places for women and girls, build strong women leaders, and advocate for women's rights and civil rights in Congress.

Women come to us in times of crisis, as survivors of rape or domestic violence. They come for job training and career counseling. They come for childcare. They come for health and fitness. They come for a variety of reasons. But they come. And they leave with a renewed spirit, new skills, and stronger lives.

YWCA associations are divided by regions nationwide. This enables our associations to pool resources, provide additional services for their clients and combine their voices for change at the national and local levels.
